Jordy Weaver, MSN FNP-C

Jordy Weaver is a Family Nurse Practitioner, board certified by the American Academy Nurse Credentialing Center, and licensed in the state of Arizona to provide family medical care.

She started her career as a nurse working home health care managing acute and chronic diseases in the home. She continued her career in home health working as a branch manager and then Director of Nursing. This experience gives her a strong insight into the challenges patients face at home and a deeper insight into managing care for acute and chronically ill patients. She continued her education obtaining her bachelors degree in nursing from Arizona State University and her masters degree from Chamberlain University.

Jordys mission statement is: As a Family Nurse Practitioner, my mission is to ensure all my patients are treated like a partner in the management of their own health, to promote autonomy and healing of mind, body and spirit.
