Once you have scheduled your appointment you will receive an email confirming your time and requesting further information, please complete the form prior to your visit.

New patients: "Sign up" for your Patient Fusion account using your email and unique username. If you're unable to log into your Patient Fusion account, please arrive 15 minutes early to your appointment in order to complete the intake form in the office.

When scheduling an appointment as a spouse or for your dependent, please include the following in the "Reason for Visit" section:

-The reason for the visit e.g., Fever, UIL Physical
-Forney ISD employee's full name
-Childs date of birth required for dependents ages 2-17
--Note: For minor dependents, please use the FISD employees DOB when submitting the appointment request.
