Rachel is a board-certified Psychiatric Nurse Practitioner based at Clearwave Mental Health's Latham office.

Rachel Hart is a board-certified Psychiatric Mental Health Nurse Practitioner PMHNP with more than 10 years of healthcare experience. She earned her Bachelor of Science in Nursing from Russell Sage College and her Master of Science in Nursing from Frontier Nursing University. Throughout her career, she has worked in a variety of psychiatric settings, helping patients with a wide range of mental health needs.

Rachel brings diverse clinical experience, including emergency psychiatric care, work with veterans, and caring for members of the LGBTQIA community. She also proudly served three years in the United States Army Reserve as a medical officer.

Rachel takes a whole-person, patient-centered approach to care, recognizing the strong connection between mental and physical health. She believes in collaboration, ensuring each patient feels heard, respected, and comfortable with their treatment plan. Her goal is to create a supportive environment where individuals from all backgrounds can feel empowered on their path to wellness.
