Your appointment will be DENIED, CANCELLED 24 hours prior to your visit if we have not VERBALLY confirmed this time slot with you.
Have your insurance info ready when we speak to you by phone. We accept Aetna, Anthem and United Health Care-Optum insurance plans.
In the space "What is the reason for the visit?" include your Insurance ID and Group Number. Your information is secure on this website.
2. RETURNING PATIENTS
In the space "What is the reason for the visit?" include your new Insurance ID and Group Number if it has changed. In this same space let us know if you already have an appointment that needs to be deleted.
