Amar Mohan MD grew up in Georgia and is a board certified internal medicine physician with a special interest in geriatric medicine dementia care and treatment psychiatric medicine transitional care and holistic medicine After completing his internship at Grady Memorial Hospital and residency with Ohio Health Dr Mohan worked as a hospitalist for Northside Hospital As a hospitalist Dr Mohan came to more fully understand the needs of our seniors and created Transitional Care Physicians of America a multipractitioner and multispecialty practice aimed at caring for patients where they live in their homes Dr Mohan continues to transform home based care and has partnered with Salude the Art of Recovery to promote the safety and improve the well being of all persons once they have been discharged from the hospital Dr Mohan continues to work to guide educate and heal his patients through their journey
