A virtual visit provided outside of normal business hours is an optional self-pay service under Florida law. These services are distinct from routine office visits and are not billed to insurance.
Normal Business Hours Monday-Friday 9:00am-430pm
Weekday evenings: Monday-Friday 5:00-8:00 PM
Weekends: Saturday mornings 9:00 AM-12:00 PM

Pricing Due at Time of Service
After-hours visit telemedicine 15 minutes: 145.00 Established Patients. 165.00 New Patients.
A Doximity HIPPA Compliant Zelle Invoice will be sent to your mobile telephone number as a text message and must be paid before your visit, otherwise you will not be seen.
Fees are disclosed in advance and may change at anytime without notice, set independently of insurance reimbursement schedules.

Insurance Financial Responsibility
By scheduling your after-hours self-pay appointment I understand and agree that:
My health insurance will not be billed for this visit.
I am personally responsible for full payment at the time services are rendered.
I will not submit this encounter to my insurance carrier for reimbursement.
I may decline after-hours services and schedule care during normal business hours without penalty.
