A Costco membership is NOT required for an eye exam.
Eye exam services: routine exams, glasses exams, contact lens exams, diabetic exams, and medical eye exams, including cataract and glaucoma evaluations.

We accept vision plans, medical, medicaid and medicare insurance. Please specify your insurance in the "Reason for Visit field" when booking online, if possible, so we can verify coverage.
Medical: Most insurances including Medicare, Tenncare, Aetna, BCBS, Cigna, UHC, UMR, Ambetter
Vision: Most insurances including VSP, Davis, Superior, Alawayscare, Avesis, Envolve, NVA, March, Spectera, Premier, some Eyemed plans
Note: Eyemed does not allow us to take all plans, however, you can use your out of network benefits and get reimbursed by Eyemed.

1. Book online by using the arrows to find a date and time. Please specify your medical and vision insurance in the Reason for Visit field, or
2. Call or text 615-379-9881
3. If you are booking for a child, you can put the parentguardian's name in the Date of Birth field and put the child's date of birth in the "Reason for Visit" field.
Walk-ins available, but appointments are recommended.
If you are having EYE PAIN, REDNESS, or CHANGES IN VISION, please call or text us at 615 379-9881. If you are having a medical emergency and unable to reach the doctor, please head to nearest emergency room.
