How to get connected:
1. Schedule a Free 20-Minute Intro Visit. You can schedule your appointment on this page by clicking on one of the available dates and times. After filling out your contact information, you will receive an email from Patient Fusion saying that your appointment request is pending. You will receive a second email when your request has been approved. You may also schedule by calling 507-474-2221 and leaving a message. We'll get back to you to confirm a time.

2. Join Your Secure Video Visit. You will get an email from Updox with a link to your video appointment. Log in 15 minutes early to test your connection.

3. Complete Consent Form. We will send you a secure link during your intro visit.

4. Set Up Your Appointments. If it is a good fit, we will help schedule your free mental health assessment and four follow-up sessions.
