I have appreciated the trust you have placed in us to provide your medical care.
Ensuring continuity of care is important, and we encourage you to establish care with another healthcare provider as soon as possible.

If you would like a copy of your medical records or wish to have them transferred to a new provider, please submit a request by February 28th 2026. Requests can be made by phone or email at - see below.

For urgent medical needs after March 30th 2026, please contact your new provider or seek care at an urgent care center or emergency department.
