Book your appointment to be seen by Dr. Celestin or our Clinic Nurse follow up appointments only!. We will confirm your appointment once the request is reviewed by one of our staff.

For insurance patients: please upload your insurance card and Driver's license or Government-issued ID front and back to the patient fusion portal and complete the indicated forms prior to your arrival. IF your insurance requires referrals, please have them ready before you schedule your appointment. Please be advised: appointment requests that have been made without a referral if required will be canceled. Watch your email for communication for us about your upcoming appointment!

For Cosmetic consults: Your appointment request places a temporary hold on your desired appointment time. Once we receive your requested appointment, you should expect to receive an invoice for the consultation fee of 100 at the email address you provide. Your consultation fee applies toward the cost of your procedure. Failure to remit payment for the consultation invoice within 24 hours of receipt will result in a release of your requested appointment time. Once payment is received, your appointment will be confirmed. Please then watch your email for further communication from us about your upcoming appointment!

CANCELLATION POLICY: We reserve your appointment time especially for you. To ensure that we have time available to see all of our patients, please be aware that we request our patients inform us no later than 48 hours prior to their scheduled appointment if they cannot make it. We strictly enforce a 50 cancellation fee in the event of: Appointment no-show, appointment late show greater than 15 minutes, cancellation less than 24 hours prior to your scheduled appointment, and same day cancellations.
