As of 2025, Dr. Songvilay has served the community of San Diego for over 29 years as an Osteopathic Physician and a Designated Civil Surgeon for Homeland Security, in the heart of San Diego, Hillcrest.  He speaks English, Lao,  Thai,  and Dai lue.  His office professional staff speaks Vietnamese.  Spanish interpreter is available through outside third party.  
His educational background began at Mead elementary school in Southeast San Diego, Pershing Junior high school,  Patrick Henry high school, San Diego State University, Des Moines University.  He concluded his medical training at the internship and residency programs for Family Medicine at Downey Community Hospital, associated with Western University,  USC, Harbor UCLA, and Children Hospital of Long Beach. 
Current services provided:
. USCIS Civil Surgeon medical services for I-693 form:
-- Civil surgeon physical examination is 295 DOLLARS; plus 165 DOLLARS for Tuberculosis blood test, Gonorrhea, and Syphilis; verify old vaccination record and status.  Vaccines are extra if required.  There is an additional 6.50 DOLLARS extra if paying with credit card.  No fee with debit card.
For USCIS I-693 exam patients:
1.  What do I need to bring?
      -Driver's License, Passport, or Government ID card
      -Old vaccine records  with English translation
      -Mental health record must be available for review, if applicable
2.   When will I receive my sealed I-693 envelope?
      -3 to 14 days when all examination is complete and normal
      -We will notify you when paper work is ready for the certification.
3.   If I am 14 yo, do I need to bring my responsible adult?
      -No
4.   Can I pay with my personal check?
      -No.  Cash, debit card, or all major credit cards ONLY.  There is 6.50 DOLLARS charge for a credit card payment.  No fee for debit card.
5.   Do I see a designated Civil Surgeon doctor with USCIS?
      -Yes
6.  Do I need to bring a current I-693?
      -No, but it would be helpful if you can print out pages 1 and 2, and fill them out 
       and bring them with you.  You may obtain a copy at the following website:

7.   Do we give a second sealed of original I-693 envelope?
      -No.  Only at the request by USCIS.  We can provide extra set of copies if needed.
8.  Can I do the medical exam remotely?
